Clinic Service Corporation (“Clinic Service”) apprises of a data incident that may have potentially affected individuals’ information. Clinic Service provides financial collection services for its clients, including Children’s Eye Physicians.

On August 17, 2025, Clinic Service discovered unusual activity on its network and immediately began an investigation. The investigation determined there was unauthorized access to certain information stored within its network between August 10, 2025 and August 17, 2025. Therefore, Clinic Service conducted a comprehensive review of the information to determine the types of information potentially affected and to whom it is related.

On December 8, 2025, Clinic Service completed its review and notified Children’s Eye Physicians. On January 2, 2026, Children’s Eye Physicians requested that Clinic Service provide notice of this incident on its behalf. The types of information potentially affected may have included an individual’s name and one or more of the following: address, phone number, email address, payment card information, date of birth, medical diagnosis/treatment information, date of service, patient ID number, medical record number, Medicare/Medicaid number, health insurance information, health insurance claim number, health insurance policy number, and/or treatment cost information.

In an abundance of caution, Clinic Service is providing potentially affected individuals with access to complimentary credit monitoring and identity protection services. If you have questions about this incident or would like to enroll in these services, you may call their dedicated call center at 1-833-580-0434, Monday through Friday between 6 am to 6 pm MT, excluding major U.S. holidays. Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports, account statements, and explanation of benefits forms for suspicious activity and to detect errors.
