[Christian County, Missouri] – On or about March 28, 2025, G5 Enterprises, LLC (“G5”) experienced a network disruption that impacted certain systems. Upon discovery, G5 immediately took action to address and investigate the event, which included contacting law enforcement and engaging third-party computer forensic specialists to assist with determining the nature and scope of the event. A thorough investigation determined that certain information stored on the network was subject to unauthorized access for a limited period of time. G5 then began a comprehensive and time-consuming review of the potentially impacted data in order to determine the type(s) of information contained within the data and to whom that information related. The type(s) of information potentially impacted varies by individual but may include name and one or more of the following: driver license number, medical condition/treatment information, financial account information, health insurance information, Social Security number, state identification number, governmental identification number, tax identity protection PIN, and payment card information.

Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors.
